Please review the steps for transferring care to us so we are able to provide excellent comprehensive care.
First, please sign a
release of records form and provide it to your children's previous medical provider. To ensure a smooth transition of care, we ask that you communicate with your current pediatric practice to request that records are transferred. Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
Second, we will review all of the medical records to determine if they are complete or sufficient for us to begin caring for your children. We must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment.

call our office if an urgent health issue comes up during the transition so we can help determine the best way to meet your child's needs.
