Sign-In Policy

Please arrive 15 minutes prior to your appointment to update your account with any changes to your address, phone number, or insurance coverage.

If your insurance coverage has changed, please bring your new insurance card and proof of identification.

At some visits, you will need to answer developmental or behavioral questionnaires. You can complete these questionnaires online before you come to the office by using our new application, CHADIS. If you were unable to complete the questionnaire before your visit, you will be able to do so using one of our office tablets

No Show Policy

At Dundalk Pediatric Associates, it is important that everyone’s time in the office is well spent.

Please be aware that we require 24 hour notification for the cancellation of routine check-ups, pre-ops and consultation appointments.

Any missed appointment (no show) without prior notification will be assessed a $25 no-show fee.

Referrals

When requesting a referral, please have the name and specialty of the doctor you wish to see, your insurance information, and appointment date. If you are calling and leaving a message, wait for the referral prompt and listen to the message.

Your referral will be processed within five (5) business days. Please allow us enough time to meet your request.

Please specify whether your completed referral is to be faxed to specialist’s office or picked up. 

Please remember that it is your responsibility to know your insurance company’s rules regarding referrals for medical specialists, labs, or x-ray requests.

Prescription Refills

Most prescription refills are sent to your pharmacy electronically. We recommend that you contact your pharmacy and ask them to send us an electronic refill request for each non-controlled medication that your child needs. These types of requests significantly reduce the risk of errors when filling the correct type and dosage of medicine.

Please allow 48 business hours for existing prescriptions to be refilled.

School/Camp/Daycare/Sports Forms

This is the time of year when we have the highest number of requests to complete forms (school, daycare, camp, fall sports, college). To better serve you in the completion of this task, please be aware of the following:

We will not complete any form that does not have the parent section filled out. By signing the form, we are verifying that we have reviewed all the information contained in the form, and that we have done the appropriate medical examination and screening tests needed to ensure that your child is cleared to safely participate in a specific activity. We cannot complete a form unless we have all of the necessary information.

Please allow 4 business days for completion of your forms. We are usually able to complete one routine form (e.g. Baltimore County Public School Form) at the time of your well child visit. You will have to leave your forms for a later return: if you present forms at a sick visit; if you have multiple forms that require completion; or, if there is more detailed information needed to complete your form.

There is a $2 fee associated with forms. This covers the administrative time needed to review relevant information and complete the form.

Be aware that a wellness assessment may be required before some forms can be completed. Before we can complete a form asking for medical clearance for a physical activity, your child will need to have a wellness assessment within 12 months of the start of the activity. If your child has annual well child visits, this requirement is usually easily met. However, some programs require an evaluation by a physician within 6 months of the start of the activity. Please be aware of the requirements for your child’s specific activity and plan a visit accordingly so that you can have the wellness assessment with the physician of your choice.

We accept most major insurances. Please contact our office for information on a specific medical plan.

Appointments

WELL CARE appointments

Well care appointments can be scheduled up to 6 months in advance.
During a well care visit, your physician may address concerns (headaches, stomach aches, etc.) that you may have or issues (earaches, sore throat, etc.) that your physician may discover which are outside the scope of a normal well care appointment.
Your physician may also address a chronic condition that your child may have (ADHD, autism, constipation, etc.).

In either case, there may be an additional charge to you in the form of a copay or a deductible (depending on your insurance coverage) since appropriate billing codes for these issues fall outside the scope of “well care” as defined by insurance companies.

CHRONIC FOLLOW-UP appointments

During a chronic follow-up appointment, your physician will re-assess the management of chronic conditions, such as asthma, autism, constipation, allergies, eczema, diabetes, cerebral palsy, or behavioral issues.

Since your child may also be seeing pediatric specialists who are co-managing these conditions, these visits enable our physicians to stay up to date with your child’s care and better assist with complications that may arise.

Your physician will determine how often these follow up appointments should occur.

MEDICATION CHECK appointments

During a medication check appointment, your physician will evaluate how your child is tolerating prescribed medications, and the effects that the medication is having on school performance and/or behavior issues.

Med check appointments may be scheduled up to 6 months in advance.

ADD/ADHD evaluations

ADD/ADHD evaluations are scheduled as a 45 minute appointment.
Your provider will have you and your child’s teachers complete Vanderbilt forms that will help with this assessment.

SICK VISIT appointments

Sick visit appointments are for all other issues that are not listed above.
Sick visits are used for acute illnesses, such as infection, injuries, sore throat, ear pain, and wheezing.

Sick visits can also be made for less urgent chronic issues, like headaches, stomach aches, and other lingering aches and pains.

We will try to arrange sick visit appointments with your child’s primary care provider on the same day, and at the earliest mutual time as the provider’s schedule allows.

Due to COVID-19, we are not providing our typical WALK IN hours.  Please call the office so we can schedule a same day sick telemedicine or in-office appointment.
